Full Job Description

The Health Care Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional.

The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The CSW will promote optimum independence and individualised client centred care to a delegated caseload.

The post holder will also support the Community Nursing/Rehab Team to deliver against key performance indicators and maintain quality of the service.

Must have 6months or more UK experience working as a HCA

Start time 8am - 16.30pm Monday - Friday full time

Basic skills wound car, insulin administration, catheter care and leg bag change, prompting medications from blister packs as we still have some patients on blister packs prompting insulins for self-caring patients

· To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.

· To undertake, and report back on delegated clinical activities that have previously been signed competent. Eg: vital signs, routine urinalysis, glucose monitoring, gait/mobility training, falls prevention awareness groups, increasing independence through activities of daily living

· To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively

· To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve

    · To liaise and refer patients to the MDT team as appropriate in accordance with local procedures.

    · To assist with errands and general duties appropriate to the clinical area as required.

    · To prescribe appropriate equipment and educate carers and patients in the safe use of this equipment, after being deemed competent to do so.

    · To use clinical judgement and skills to report any untoward changes in carers or client's health status immediately to the relevant team member, GP, District Nurse or emergency services if appropriate.

    · To collect data from patients/carers for recognised outcome measure and document for therapists/nurses to interpret.

    · To provide care to patients with challenging behaviour by maintaining a calm safe environment and reporting back to the nurse/therapist as indicated.

    · To plan and implement straightforward tasks and work guided by standard operating procedures and protocols and undertake roles/duties as delegated by a nurse/therapist.
