Full Job Description

To provide in conjunction with the Health Visiting team an accessible, person centred, effective needs led public health nursing service to the 0-5 age population and their families. To participate in service development and health improvement activities. To work in collaboration with other health care and service providers, including Children centres and Early years settings to identify and address health needs. To monitor, maintain and develop the service. To deliver the role specific elements of the Universal Healthy Child Programme (DH 2009)
The post holder will be responsible for:

Working hours Mon - Fri
Must hold previous experience working as a Community Staff Nurse in the 0-19 team, · To organise and prioritise own work without direct supervision, reporting any issues, concerns or personal limitations to the Team Lead.
To be responsible for the care and safe keeping of equipment issued for personal use and to report any defect or loss.
To facilitate and maintain a good working relationship with GP's, Children Centres, Early Years Providers and other health and social care service providers, so as to provide a comprehensive service to children and parents.
To attend and actively participate in team, locality and other relevant meetings., To identify and record all new problems and concerns and report back to the Team Lead or named Health Visitor
To liaise with the Health Visitor when the child's/parent's needs are beyond their own role or scope of practice
To provide care and attention to the needs of vulnerable children which includes risk assessments and taking appropriate action as delegated by the Health Visitor
· Participate in managerial supervision i.e., 1:1s
· To provide skilled, effective and evidence-based public health nursing to children/families and to operate safely and in line with best practice guidance.
