Full Job Description

The District Nurse (DN) is a leader within the nursing team. The post holder will be expected to undertake the holistic assessment of patients using a person centred approach, and co-ordinate care packages across both health and social care as appropriate to avoid inappropriate admission to hospital or long term care environments, in addition to facilitate the timely discharge of patients from secondary care whilst taking active part in innovative projects that enhance patient centred service delivery and development. The DN is responsible for promoting good teamwork and providing teaching and support to other team members. The post holder will work in collaboration with the designated District Nurse Team Leader in managing the care of patients with long term conditions, incorporating monitoring progress and promoting the preventative aspects of nursing practice.,

  • To provide a comprehensive nursing service to patients in a variety of community settings, such as the patients' own home, health centres/clinics, Residential homes and GP surgeries.
  • To take continued responsibility for the assessment, planning, evaluation and where appropriate implementation of nursing care to individuals, groups of patients and their families as defined through GP attachment and specified geographical area.
  • To act as the patients advocate and, by providing information and support, facilitate the patient's own choices with regard to nursing care, promoting independence and self-care as appropriate.
  • To administer medication via Intravenous routes and set up syringe drivers and to teach and competency assess others in these skills.

    £22.92 an hour Weekly pay will include: Basic pay + HCAS + 12.07% Holiday pay
