Shropshire Community Health NHS Trust has an exciting opportunity for a Band 5 Community Staff Nurse to join the South West Neighbourhood Team (ICS/IDT).
We are a proactive and forward-thinking team, who constantly strive for the delivery of excellent patient care and modern ways of working. The South West Community Nursing Team works closely with a variety of teams, including Therapy, Virtual Ward and Rapid Response supporting multi professional working.
The service operates between the hours of 8am -6pm and is a 7 day, 365 days a year service. The Community Staff Nurse role does involve working regular weekends and some Bank Holidays. As this is a community post, the successful candidate will need to be able to meet the travel requirements of the post; please demonstrate on your application form how you would meet this requirement.

You will need to be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development. We actively encourage newly qualified nurses to our team and in return we will offer a preceptorship programme and support professional development in a nurturing environment allowing you to reach your full potential.
The team provides nursing care to patient within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
You need to be an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team.
You will be will be accountable for the delivery of high quality care to patients whilst working with other members of the MDT to promote independence for patients on the team caseload.
Shropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford and Wrekin, and some services in surrounding areas too. These range from District Nursing, Health Visiting, Virtual Ward and Community Hospital's.
