Full Job Description

We are seeking to recruit a specialist Duty Worker for 3 days per week to work as part of the Adult Community Mental Health Service based in Watford. This is an exciting opportunity for an experienced, Registered Nurse, Social Worker or Occupational Therapist. The post holder will be based at Colne House and the role will be spread over 3 days (this can be decided by the postholder). Main duties include but are not limited to; managing and responding to requests from service users, carers and other stakeholders for urgent support or advice. This may involve undertaking face to face initial assessments and / or welfare visits and the assessment of risks and liaison with other HPFT services and teams. This is both a challenging and rewarding position where good communication and organisational skills are required. Support and advice is provided by the Duty Manager who you will be working closely with and other senior staff including the medical team. Applicants should be able to demonstrate the ability to work well within a team, prioritise and manage own workload and display initiative whilst maintaining a positive, professional and calm approach. The Adult Community Mental Health team is a large and busy team providing care and support to Adults of working age who are experiencing mental health difficulties that are impacting on their lives and daily functioning. The service operates Monday - Friday, 9am - 5pm. Preferred Car driver with access to a carMain duties of the job The post-holder will be responsible for the clinical and operational day to day service delivery ensuring that an individualised comprehensive assessment and Care Management service is effectively delivered. Responsibilities include the clinical and operational supervision of junior nursing staff and students as delegated by the manager. To be accountable for delivering a quality service that is effective and efficient in line with the organisation's objectives and performance indicators. To contribute to strategic service development working collaboratively with other agencies., Undertake comprehensive health needs and risk assessments. (including capacity assessments) and develop a care plan (Health Action Plan) in accordance with identified needs establishing specialist course of responsibilities/actions based on Recovery Principles. Carry out joint assessment with other MDT as required. Aware of and comply with the relevant mental health and work related legislation and policy at all times. Maintain patient safety, in collaboration with patients, carers and other team members Responsible for recognising the potential for or signs of patient harm, abuse or neglect and taking all reasonable steps to protect the patient. Provides clinical advice on complex issues to other members of the clinical team and staff from other disciplines. Undertakes urgent and priority assessments with risk assessments Liaise with other services eg: Police; A & E as required Provide relevant up to date evidence based information about treatment/intervention that enables service users to make informed choices about their care. Deliver evidence based care in a timely manner. Make reactive recommendations as necessary when working alone in the community. Undertake health promotion activities with individuals and groups. Promote physical health and well-being by providing health checks/screen and monitoring. Where necessary to administer prescribed medication or to supervise its administration and assess the service user for desired and non - desired effects.   • Car driver (unless you have a disability as defined by the Equality Act 2010, which prevents you from driving), RMN or Dip/BSc (Hons) / MSc in Mental Health Nursing
  • ENB 998/Preceptorship and Mentorship
  • Professional Registration, Strong experience at Band 5
  • Experience of working in acute mental health inpatient/day care services, You must have appropriate UK professional registration.
