Full Job Description

  • To work as a member of a multidisciplinary team assisting in the provision of
  • care, support and rehabilitation to adults requiring intermediate care.
  • Responsible for the care of the patients on the ward.
  • Delivery of Quality Patient Care.
  • Progression of Patient Rehabilitation.
  • To be responsible for the care of a group of patients during the shift.
  • Assists in meeting the requirements of the Clinical Governance programme.

  • To undertake tasks in delivering patient care and rehabilitation as directed
  • by service professionals
  • To progress patient treatment, following initial assessment and care
  • planning by an appropriate service professional. To observe any changes that affects the individual and accurately communicate and document these back to the appropriate team member.
  • To take responsibility for the well-being and comfort of patients. To
  • observe an individual's ability to tolerate and manage their treatment programme. To alter, adapt or stop activity in response to the individual's ability and state of health.
  • To undertake physical measurement monitoring as directed (e.g. blood
  • pressure, oxygen saturation, heart rate etc) and report findings to the appropriate service professional.
  • To communicate effectively and professionally at all times with patients,
  • carers, service professionals and other health and social care colleagues as appropriate. Page 2 of 6
  • To reflect on and evaluate one's own practice in order to improve the
  • delivery of patient care.
  • To ensure tasks, therapy and care are only carried out within personal
  • competency and that personal competency is maintained and developed.
  • To be responsible for organising, prioritising and planning own workload to
  • meet patient and service priorities, readjusting plans as situations change or develop. Clerical Duties
  • To share responsibility with all team members for the day-to-day smooth
  • running of the service, making specific contributions as requested.
  • To participate and assist with service audits as required.
  • To record and collate statistics as required.
  • To maintain clear, comprehensive and accurate clinical records and to
  • manage all documentation processes (e.g. reporting of clinical incidents / near misses). Other Responsibilities
  • To undertake any other such duties as may be required from time to time,
  • that are consistent with the responsibilities of the grade.
  • To comply with the Trusts and departments policies, guidelines and
  • procedures.
  • To attend all mandatory training as required in line with Trust employment
  • and professional need.
  • To actively participate in team meetings, adding value to the department's
  • business planning, service developments and clinical governance initiatives.
  • To participate in performance and development programme and to be
