Full Job Description

+ Work with patients who may have complex and varying physical and psychological presentations
+ Work independently with patients who have been seen by the therapy team to continue and progress treatment programmes and feedback sessions to the therapists. This may include teaching patients to get in and out of bed, walk with different walking aids e.g., a crutches, helping them wash and dress, or assessing their function in the community
+ Have an in-depth knowledge of a specific care pathway for patients. Work independently with these patients, to assess and treat them. Monitor and evaluate the progress of these patients and highlight to therapy staff when the patient deviates from that care pathway.
+ Work with therapists, nurses and other assistants to assess and provide goal-directed clinical intervention to patients on the ward and in the patient's home.
+ Run exercise sessions or activity groups, organising patients to attend, and coordinating transport to assist patients in and out of the hospital.
+ Assist patients' home from hospital and assess that they are safe in their own environment. Set up equipment as directed by the therapist to aid their function
+ Carry out and progress treatment sessions with patients in their own home, reporting progress and any concerns to the therapist, The person specification below is not the full person specification, but outlines the criteria against which your application form will be assessed.
