Full Job Description

The roles will involve working in busy, challenging and rewarding environments in Community settings, in line with the overall objective of the service As part of the Integrated Provision, the post holder will work in partnership with the Early Intervention Community Team (EICT) in the prevention of inappropriate hospital admission, the facilitation of hospital discharge and the provision of home based treatment in line with the overall objectives of the service. A car driver and access to a vehicle are essential,
