Full Job Description
- To provide highly skilled and autonomous care to a designated caseload including takingresponsibilityfortheassessment,management,andevaluationofevidence-basednursingcaretopatientsin avarietyofcommunitysettings. · Act as the Team Leader for the Rapid Response Nursing Team on a rotational basis responsibleforday-to-daymanagementoftheservice; acting asakeypointofcontactforGPsandothergeneralpracticestaff. · Toworkwithinandacrossprofessional andorganisational boundaries,liaisingwithGP's other professionalsandstatutoryandvoluntaryagenciestodeliverindividualisedand expertnursingcaretoahighstandardtoclient,andleadingcommunicationwithall stakeholders. · Take designated responsibility for discrete areas of operational management/ clinicalpractice. · Work on a rotational shift basis - 8am - 4pmand2pm-10pm, when working on the LAS/UCR car shift is 8am - 6pm all services are7dayaweek including bank holidays.
- TosupporttheService Manager/Clinical Leadand colleaguesindeliveringhighqualitycarethatdeliversbestvalueandmeetstherequirementsofcommissioners. · Toprovideskilled,effective,andevidence-basednursingcaretopatientswith complexcareneeds,in avarietyofcommunitysettings. ·Utilise advanced clinical and diagnostic skills and expert knowledge to carry outthoroughassessments of physical and psycho-social needs todevelopa comprehensive short term care plan aiming to optimise health and wellbeing that will preventahospitalattendance/admissionorallowearlydischargehome.Toincludetheneedsofcarers. ·Provide direct advanced, autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/ or complex health needs and long-termconditions. ·Work within Clinical Guidelines, Policies, Procedures and Patient Group Directions (PGD's)utilisingexpertjudgmenttoworkoutsidetheseframeworkswherenecessary. ·Prescribe as an Independent Nurse Prescriber as required, within professional competence, ensuring that all duties in relation to medicines management meet requiredprofessionalandCLCHstandards.
