Full Job Description

The Bank Palliative Care Specialist Nurse (CNS) will join an established Specialist Palliative Team which provides a 7 day service.

The post holder will be expected to carry out a comprehensive holistic assessment of need for patients, with a palliative diagnosis, using their clinical reasoning and expert knowledge to ensure the delivery of quality clinical outcomes working in close collaboration with other health and social care professionals.

The post holder will be expected to contribute to the wider palliative and end of life care agenda within the community in Luton, provide education across a range of professionals and ensure the team provides a flexible responsive service that meets the local needs within Luton.

Work in partnership with other providers to deliver a seamless service across all care settings.

Provide support and be proactive in specialist education and training to other professionals, involved in palliative and end of life care in the community setting.

Act as a role model for palliative and end of life care within the organisation demonstrating high standards of care and providing clinical leadership to others.

To review local and national strategy for palliative and end of life care and support local implementation of changes.

Salary will be subject to the 2024/25 Afc increases once an implementation date is agreed.
