Full Job Description

Band 6 Memory Assessment NurseAs a Memory Assessment Nurse, you will deliver high-quality memory assessments, contributing to the early diagnosis of dementia and supporting patients and families through the diagnostic journey. You will manage a community caseload, work collaboratively within a multi-disciplinary team, and provide person-centred care to enhance the lives of those living with dementia.

Band 5 Primary Care Dementia Practitioner (PCDP)In this role, you will be a key link between primary care and mental health services, supporting patients and their families from diagnosis through ongoing care. You will manage a community caseload, provide education and advice to caregivers, and work closely with primary care professionals to ensure integrated and effective support.
