Full Job Description

To provide high standards of care to patients with health and social care needs. Care will be delivered within the patient's own home / other community setting with an enablement focus where appropriate. The post holder will be required to deliver care, under the supervision of senior support workers or therapists, following a prescribed care plan and reporting back to other members of the team, the patient and family/carers as appropriate highlighting any concerns or changes in patient's condition / abilities. The post will involve lone working. Senior support workers / Therapists will provide necessary support and there is access to oncall provision out of hours.., To communicate professionally and effectively, both verbally and non-verbally, with patients and their carers. This may include communicating with patients and carers who are deaf, visually impaired, have poor memory, cognitive problems or for whom English is not their first language. To use skills of persuasion, motivation and explanation to encourage patients and carers to participate in enablement tasks where appropriate which may be painful, challenging and / or time consuming- and therefore require negotiation.

  • To convey appropriate to patients and carers in a sensitive and timely manner and using language they understand.
  • To work within the team maintaining close liaison with all Quickstart / Maximising Independence staff to ensure the smooth running of the team and the effective delivery of the service to patients.
  • To communicate professionally and effectively, both verbally and non-verbally, with your colleagues and other health and social care colleagues, voluntary agencies and carers to expedite continuing care of patients.

    £14.26 an hour Weekly pay will include: Basic pay + HCAS + 12.07% Holiday pay
