Full Job Description

The main responsibilities of the post holder are:

1. To prioritise workload, delegate and supervise more junior staff to aid provision of high standard of patient care.
2. Co-ordinate patient flows in and out of hospital maintaining the highest clinical standards.
3. To support the wards, in ensuring that they have in place a System to capture a real time bed state of their admissions, discharges and predicted discharges, such as the Midnight bed return and capacity management system (CMS).
4. To have a wide range of knowledge in approaches to communicating and managing patient care.
5. Ensure junior staff are actively supported to enable them to achieve their learning needs as discussed in appraisals and regular supervision sessions.
6. Ability to travel effectively to acute hospitals to complete face to face assessments when appropriate., This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735 p.a. pro rata for part time).

The Intensive Rehabilitation Service is a large multidisciplinary community team supporting patients to recover after illness. As the Band 6 bed assessor within IRS, you will support the bed management team to screen all patients accepted into Meadow Court inpatient rehabilitation beds. This will also involve face to face assessments within our acute partner hospital sites, assessing complex patients to support the patient pathway from hospital discharge into NELFT's community services. There is an expectation to join the weekend rota. You will support the other staff members, provide training, supervision and joint sessions. The role would be suitable for therapist or nursing staff.
