Full Job Description

We are recruiting for a Bereavement Officer who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff., To assist in providing a liaison service and advice to bereaved relatives in matters connected with deceased patients. The post holder will ensure that patients and their relatives, in the case of a death, are dealt with sensitively, courteously and diplomatically at all times.
The Bereavement Office is a close knit team of 3 - who sensitively, effectively liaise and support the Bereaved at a very difficult, and sad time.
We work in close collaboration with the Medical Examiner's Office, the Mortuary, Funeral Directors, Registry Office and the Coroner's Office (Where indicated).
We live and breath the Patient First Ethos and this does not cease once death has occurred and continues till care can be transferred to their final destination

The post holder will ensure that patients and their relatives, in the case of a death, are dealt with sensitively, courteously and diplomatically at all times.
To provide administrative support to Bereavement office and assist in the preparation of all paperwork, update and maintain central bereavement database (EDEN)
To update, amend and add patients identifiable onto the EDEN/PAS
To co-ordinate the collection of patient valuables from the wards and keep accurate records,
To co-ordinate the return of personal belongings to relatives in accordance with patient property policy
To take calls in the department and deal with them efficiently and within time frames setting out, providing advice on registration and funeral arrangements.
Contact relevant medical staff to arrange for documents to be completed if instructed by the Medical Examiners Officers
To received Medical Certified Cause of Death (MCCD), and scan to relevant people
To ensure that Cremation forms are completed in a timely fashion and scanned and forwarded onto the relevant Funeral Directors for actioning
To liaise with the mortuary and support viewings of the deceased for bereaved relatives.
To liaise with the funeral directors when required, to act as a point of reference for issues to be addressed
To ensure trust policies and procedures are adhered to at all times
