Full Job Description
An exciting opportunity has arisen for a part time Band 5 Bone Health Nurse to join Nottingham CityCares Bone Health Service. The team operate Monday to Friday, 08.00- 16.00, this post will be for 15 hours, and we can consider flexible working for example school hours. The team are based at Bulwell Riverside.
The Bone Health Service aims to be a proactive, forward thinking, and dynamic service which provides evidence-based assessment and follow up for individuals requiring ongoing Osteoporosis management in the community and those who are at risk of frailty fractures., To provide assessment and management of patients who have osteoporosis and bone health conditions.
To work autonomously delivering high quality evidenced based care to patients seen in their own home or in a care home setting.
To administer IV and injection therapy in patients homes and care homes.
To facilitate and autonomously deliver osteoporosis therapies in the community.
To work in partnership with GPs, Community Geriatricians and Specialist Secondary Care Physicians in the management of patients within Nottingham City who have osteoporosis and bone health conditions.
