Full Job Description

An opportunity has arisen within Endocrinology and Breast Surgery department for a motivated and dedicated MDT Co-ordinator & Breast Data Manager based at St Bartholomew Hospital.

Reporting to the Service Delivery Manager and working closely with the Management and Clinical teams, the post holder will be responsible for ensuring that deadlines are met and that there is a pro-active co-ordination of cancer patient pathways to ensure appropriate treatment is received in line with national and local targets.

The post holder will establish key working relationships with all members of the administrative and clinical teams including nursing and medical staff, allied health professional and support workers. They will act as a point of contact (in phone and in person) in communications with patients, relatives and GPs and other external hospital staff / tertiary referrers.

You will be responsible for providing waiting times and cancer outcomes data, monitoring, and data collection (including collection and tracking of notes). You will also be responsible for weekly MDT meeting, of which you will prepare facilitate and record outcomes of each patient discussion via the Somerset cancer database in a live environment. As an MDT coordinator you will ensure concise and daily tracking of each step in a patient pathway, data quality and completeness, expediting appointments/visits ensuring patients are not delayed at any point in their pathway., This post will be split up to provide dedicated administrative support to the Breast Surgery Unit & Endocrinology multi-disciplinary team (MDT), working to provide high quality care for patients with cancer. The post holder will be expected to ensure the MDT runs smoothly and to time. Post holder will be required to live type at the meeting, which will be signed off by MDT Lead. They need to ensure all patients are discussed within agreed timescales, and treatment plans are in line with CWT guidance and constitutional standards.

The post holder is to actively manage the administrative processes within the Breast Unit at St Bartholomew's Hospital. To actively capture and manage data for the Breast Service. To manage patient pathways, including RTT validation and coordination of specialist MDTs, ensuring a smooth and efficient service for patients throughout treatment.

Key working relationships are with wide range of MDT professionals, patients & their carers, GPs and members of the internal administrative team and clinical teams including managers, administrators, nursing and medical staff, allied health professionals and support workers. They will frequently liaise with a range of staff internally across the sites/Trust and with partner and external NHS Trusts.
