An exciting opportunity has arisen to join the Pennine Lung Service as the Breathlessness and Long Covid Service Co-ordinator
The post holder will support the Breathlessness and Long Covid MDT Clinic and oversee the entire Multi-Disciplinary Team process both pre and post MDT. Long Covid is now a recognised illness following the Covid-19 pandemic. This role will be essential in delivering the MDT for patients who require Tier 3 services across the NCA.
We require a patient focused individual who has excellent people skills, can communicate appropriately with both health care professionals and patients, and who is organised, proactive, and passionate about improving outcomes for patients., The post holder will be expected to work across a range of IT systems namely excel, PAS, SCR and CRIS, to coordinate booking of patient appointments into the MDT and ongoing referrals. The post holder will work both directly and indirectly with both patients and their carers to assist in coordinating the care of patients through their hospital care journey, identifying, escalating, and managing issues that patients may have with transport and interpreters as appropriate. The role will be key to the planning, uploading of documents, taking notes and actions at the MDT, and informing the patient and GP of the outcomes post MDT. They will also need to tackle any actions and ensure they are completed by health care professionals in a timely manner.
