Full Job Description

The Single Point of Access (SPA) Administrator is crucial to the whole service as it determines the timely response to referrals, guaranteeing the safe, appropriate management of patients throughout Brent Unplanned Care services. The SPA is an administrative service whose purpose is to streamline and coordinate all referrals to BRR, providing a central base through which referrers can access the service. The Administrator coordinates the whole service pathway and monitors the process. The Administrator is an integral team member and is vital to the smooth day-to-day running of the service. Must maintain professional practice standards, ensuring statutory requirements are met within the framework of the Community Care Act and Council Policies and those of the Acute Trust. Be an active member of the SPA, ensuring there are safe and appropriate pathways between all agencies.,

  • The Post Holder will be able to triage referrals, distinguishing between urgent and less urgent referrals within the service pathways and ensuring that every referral is directed to the appropriate pathway. This is a crucial element of SPA as it determines the safety and appropriate, timely care of all patients. Training for this will be competency-based.
  • The Postholder will lead in liaising with all health and social organisations and updating them on any potential changes in the care management plans.
  • Provide sometimes complex information to include patients and carers. Also, request assistance, especially when there are barriers to language and understanding. This may include gaining informed consent in sensitive circumstances and demonstrating empathy with patients, carers,and families.
  • Be proficient with IT systems, competent in sending relevant information and documents to other professionals/disciplines, and able to communicate by phone, writing, email, etc., often in the context of frequent interruption.
  • Maintain confidentiality of information consistent with legislation and Trust policies.

  • The Postholder will have excellent communication skills and be able to take oral referrals, mainly by telephone, from GPs and other organisations. This will require exceptional listening skills and interpreting accurate, sometimes complex information.
