Brokerage Officer (Adults)

This role is starting asap, working Mon to Fri up to 36 hours a week in an ongoing contract.

As a Brokerage Officer you will be responsible for researching and sourcing with a person-centered approach, cost effective placements for all cohorts of service users from 18+ who require a range of services such as residential/nursing, homecare, day care, outreach, supported living and respite.

You will be required to provide day to day delivery of brokerage services, working closely with various stakeholders/professionals such as providers, social workers, both adults & children's, hospital teams, finance, revenue & benefits, legal etc.

You will be expected to communicate effectively, ensure the best value is always achieved and maintain a courteous and professional attitude.

You will be responsible for the following:

Actively covering duty Monday to Friday 9-5pm

Negotiating cost effective, best value placements in line with LBR benchmarking, contracts, and regulated providers

Brokering services for service users, based on individual assessed needs to ensure best outcomes are met.

Working closely with hospital social workers to ensure timely discharges from hospital to prevent bed blocking.

Work closely with community/locality social workers to ensure appropriate services are brokered, always ensuring safety of service users.

Working mainly within adult's older people brokerage however may include both adults & children's services for individuals with mental health, learning disabilities, autism, physical sensory and brain injury.

Liaising in an effective and timely way with team members from contracts, quality assurance, commissioning, and other colleagues across the local authority.

Liaising with providers and other parties to resolve service delivery issues and feed into safeguarding's and freedom of information requests.

Your ability to pay attention to detail and work with minimum supervision will be an advantage, as will knowledge and/or experience of working with Liquid Logic, ContrOCC and Care Quality Commission

We are looking for someone who is highly numerate, and able to communicate effectively both verbally and in writing. You will need to be able to work under extreme pressure, flexibly and contribute positively to team performance.
