Full Job Description

An exciting opportunity has arisen with the Bristol and South Gloucestershire Brokerage team within the ICB. We are looking for a positive and enthusiastic individual to join our vibrant, fast paced Brokerage Team.
This is an office-based role at 100 Temple St, Bristol BS1 6AG. The ICB has a Hybrid Working policy in place meaning some home-working is supported. However, flexibility is required by the post holder in this role and, the post holder must be able to commit to being available to attend the office at short notice when requested., The successful applicant will work with commissioners and operational teams to manage the procurement and contracting for individuals who are eligible for NHS Funded care.,

  • To support the procurement of care for CHC eligible individuals, following guidance on clinical needs, and in doing so deliver appropriately commissioned care placements and packages of care.

  • Work across all service user groups eligible for CHC and Children's Continuing Care, including older people, disabled adults, people with learning difficulties, people with mental health needs, children and young people.

  • Communicate effectively with care providers to identify suitable CHC care packages and placements.

  • Update systems to provide oversight of available care market capacity.

  • Issue provider contracts and individual placement agreements for CHC funded placements and care packages

  • Engage compassionately and professionally with service users, families and representatives when identifying care packages and placements.

  • Act promptly to address identified risks in care provision, or where changing needs require alterations to the way that care is delivered.

  • Maintain accurate information systems to evidence the effectiveness of the service.

  • Develop good links with CHC operational teams, acute hospitals, and the three local authorities.

  • Deliver against personal objectives, achieving quality outcomes, prioritising own workload, and working to tight deadlines.

  • Propose changes to own work, or that of the wider team, to achieve a more effective service.
