Full Job Description

Oxford Health and Buckinghamshire are leading the implementation of the Community Mental Health Framework to improve patients experience of care and plug the gap between Primary and Secondary Care Services. These unique roles willempower patients aged 18+ years to access timely and evidenced based support from Mental Health service.

The post holder is integral to accomplishingease of access to mental health care for Buckinghamshire's population at primary and secondary care level in an efficient and timely manner. The successful applicant(s) will facilitate improved opportunities for joint working with Buckinghamshire's voluntary care sector, whilst contributing to theleadership and development of personalised care , transformation agenda and Integrated care systems (ICS).

As part of the Bucks Mental Health Gateway Multi-Disciplinary Team (MDT) your role will provide mental health meaningful assessment, signposting and professional advice to patients , their family and your colleagues. You will work with professionals to facilitate brief intervention for patients, to enable recovery within the community, whilst recognising the value in your skills to ensure prompt access to secondary care specialisms as appropriate.

Oxford Health NHS Foundation Trust provides physical, mental health and social care for people of all ages across Oxfordshire, Buckinghamshire, Swindon, Wiltshire, Bath and North East Somerset.

This role carries an incentive premium of £3,500 which will be paid in instalments over an 18-month period. Please note that this scheme is not open to existing employees of Oxford Health and is subject to a claw back clause.
