Full Job Description

Exciting new role B6 Camhs Crisis Liaison Clinician joining our Camhs Crisis pathways expanding team. The post holder will be required to work on a shift pattern, to assess patients across the urgent care pathway, working autonomously and flexibly to manage the day-to-day assessments and/or advising on management of CYP under the age of 18 years and their families presenting in the A&E Barnet General hospital or North Middlesex University Hospital with self-harm and acute mental health difficulties. The post holder will be required to undertake a range of therapeutic tasks individually and jointly with members of the multi-disciplinary team. To develop & maintain specialist clinical skills and expertise in the assessment of CYP psychiatric emergencies and deliberate self-harm. To receive and assess referrals from A&E, pediatric/general wards, Community Teams, schools, and health colleagues across the NCL as well as self-referrals from CYP and their families. To offer advice on management of patients with mental health issues and non-organically based physical symptoms. Identifying regular attendees at Acute Hospitals with mental health problems and in liaison with Community CAMHS teams to make available current care plan as and when appropriate. There is a need to use effective and efficient IT skills in the recording of clinical information and in communicating with colleagues from CAMHS and allied service, To participate as an effective member of the MDT and be responsible for the development and implementation of a patient centred approach to care: assessing, developing, implementing and evaluating the care needs of patients and their carers To meet the physical, psychological, social and cultural needs of the individual and their family, using evidence based knowledge and skills to maintain the individual's identity. Apply clinical knowledge and experience to provide education for patients to develop a greater understanding of their mental health condition and working in a patient centred way with clients to help develop their awareness of their condition, life skills and coping strategies. To recognise the needs of the family/carers, ensuring their involvement in care where appropriate whilst offering support and education with empathy. To undertake initial mental health and risk assessment of CYP who present with high risk and/or following acts of deliberate self-harm. To independently and/or jointly carry out assessments of the mental health needs of children and young people within a therapeutic framework and to contribute fully to the care planning process. To provide a high standard of care for people in the care group in keeping with the philosophy and operational policies for the Liaison Team and in addition to the Trust's policies, practice, and procedures.
