Full Job Description

Liaising with out of area CAMHS.

Completing a desktop review of information or assessments already provided to consider the best intervention for the young person, if this did require to be commissioned (Therapeutic Needs Assessment).

Completing face to face therapeutic needs assessments.

Supporting placements to prevent placement breakdown.

Working /liaising/ reviewing with independent therapeutic services.

Maintain an overview of young people through Complex Care Panel.

Consulting, liaising, and networking with Independent Fostering Agencies- Supervising Social Workers.

Liaising and supporting with discharge planning

Supporting The Croft Childrens home.

Offering training to professionals and foster carers.

The successful candidate will ensure that Wakefield children in care are offered the right mental health support whilst not residing in the Wakefield area. This is done by meeting with the young person and assessing their needs. The successful candidate will be attending the Complex Care Panel meeting to discuss children and young people that may need assessment. At times, this role will require you to travel to see children and young people nationwide.

You would be responsible for working collaboratively with out of area social care services, foster parents/carers and other organisations.

To apply for the role, you need to have a good understanding of child development, trauma informed care, risk assessing, types of therapeutic intervention and safeguarding.

This role will also have some leadership duties within The Croft a 2 bedded therapeutic childrens home, based in Wakefield.
