Full Job Description

As the Cancer Care Co-ordinator, the successful candidate will play a key role in helping with the delivery of cancer targets for the trust by ensuring that the journey of each cancer patient runs smoothly and is progressed in a seamless and timely manner., To support the Upper GI CNS team in delivering a high standard of support to patients with cancer. This will involve being in daily contact with patients where needed. The post-holder will be expected to support and signpost patients over the telephone and progress their care pathway in liaison with other colleagues and departments in the hospital. The post-holder will also be expected to be involved with Audit and Peer Review processes.
The post holder will need to be flexible and self-motivated with a good attention to detail and the initiative to work independently as well as a team. This post is key in providing high quality information and ensuring patient's progress effectively on the cancer pathway.
The post holder will work closely with staff across the wider trust including clinicians, general managers, deputy managers, nurses, secretaries and other administrative staff.
They will support Direct To Test (DTT) pathways, managing the results spreadsheet and supporting the DSM/SM to meet our FDS target every month.
The post holder will be expected to type CNS clinic letter, declined procedure letters (supporting FDS) and review/action CNS clinic notes to ensure actions are completed.
Attendance of the weekly UGI MDT is necessary to take notes of the CNS actions. The post holder will also be direct cover for the UGI MDT Coordinator in times of absence.
