The main purpose of the Macmillan Cancer Care Co-ordinator is to work as part of the Primary Care Network (PCN) teams to proactively identify and work with people with cancer to provide co-ordination and navigation of care. A key element of this role will be to support the holistic needs assessments/cancer care reviews of patients, compiling care plans and administrating the on-going referrals to different primary, community, and specialist teams.
The post holder will be committed to providing high quality compassionate care to all patients in a sensitive and empathic manner both directly and indirectly. Therefore, excellent communication skills, and a good understanding of cancer pathways and knowledge and skills to navigate patients through the health and social care systems.
You will act as a single point of contact for cancer patients across the PCNs, and with the secondary care sector; and collaborate with key stakeholders (e.g., health, social care, welfare and benefits and voluntary organisations).
