Full Job Description

We have a number of exciting opportunities for Cancer Care Co-ordinator's to join our clinical and medical oncology teams.
There are positions in the Gynaecology and Urology specialities as well as other disease groups that are yet to be confirmed.
1 x Part Time position available (Urology)
1 x Full Time (Disease group TBC)
The successful candidate will be passionate about making a difference to patients diagnosed with cancer. If successful you will be offered an opportunity to work within one of the specific oncology specialities across the clinical teams.
Excellent communication, organisational and prioritisation skills are essential., The post holder will work alongside the disease specific CNS /ACP teams, their responsibilities will include
+ Assisting the CNS /ACP team with the facilitation and implementation of Holistic Needs assessment within the disease group.
+ Responsible for triaging phone calls and emails to the CNS /ACP service.
+ Providing support with administrative services including referrals to Macmillan and District Nurses.
+ Attend relevant clinics with the CNS/ACP team and interact with patient and relatives.
+ Willing to undertake relevant training days within the disease group and cancer pathway.
+ Willing to attend relevant MDT sessions if needed.
+ Signposting patients to relevant services, + Provide administrative support to the ACP/CNSs.
+ Book and outcome telephone clinic appointments
+ Maintain the patient Database and ensure all of the information is up to date and accurate
+ Order supplies and patient information as requested by the CNSs. Assist the CNSs with the management of the patient pathway ie ensuring that appointments have been requested and booked.
+ Organise and prioritise the designated workload in order to meet the needs of the service. Assist the CNS with handover of patient care to other teams in order to facilitate safe and effective transition of care. Communicate effectively with a range of patients and health and social disciplines. This may include a variety of methods, such as electronic or in person.
+ Provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.
+ Triage telephone and email enquiries, using a risk assessment framework.
+ Ensure patients/carers know how to contact the service when their condition / needs have changed, including out of hours.
+ Provide general information and support about cancer services, using Trust approved literature. Sign-post patients and relatives/carers to appropriate services such as Maggie's and Macmillan in order for them to speak to professionals who are able to discuss issues knowledgeably and impartially.
+ Demonstrate the ability to recognise situations that require rapid response from a health care professional (ACP, CNS or doctor) and respond appropriately. Evaluate outcomes of care delivery with the registered practitioner.
+ Act as an advocate and facilitator for patients; linking with CNSs to resolve any issues.
+ Ensure accurate documentation of patient care in accordance with hospital policies and standards.
+ Assist the CNS's with the roll out of Holistic Needs Assessment and electronic patient outcomes (eproms) within the disease group
Professional/Clinical Leadership:
· To act as a role model to colleagues, and be an effective resource to patients, carers and colleagues.
· Support clinical developments and projects to promote excellence in patient care.
· Attend and participate CNS Away Days and Strategy Meetings
· Recognise personal limitations and work within parameters of job description, or as guided by CNS. Seek help appropriately.

The Christie is one of Europe's leading cancer centres, treating over 60,000 patients a year. We are based in Manchester and serve a population of 3.2 million across Greater Manchester & Cheshire, but as a national specialist around 15% patients are referred to us from other parts of the country.
We provide radiotherapy through one of the largest radiotherapy departments in the world; chemotherapy on site and through 14 other hospitals; highly specialist surgery for complex and rare cancer; and a wide range of support and diagnostic services. We are also an international leader in research, with world first breakthroughs for over 100 years.
We run one of the largest early clinical trial units in Europe with over 300 trials every year. Cancer research in Manchester, most of which is undertaken on the Christie site, has been officially ranked the best in the UK.
The clinical teams consist of a full MDT with a specialist focus on the disease group speciality and associated treatments., + To participate in annual appraisal and accept personal responsibility for growth and development within the role.
+ Ensure compliance with Trust essential training.
+ Attend educational meetings and conferences, appropriate to the role, in order to keep up-dated with current best practice.
+ Together with other professional colleagues, develop information and resources for patients and their carers.
Research and Audit
Assist CNSs in audit and evaluation of service delivery and help identify areas for improvement and change
