Full Job Description
An exciting opportunity has arisen within the Cancer Services department to support the delivery of faster diagnosis for patients on a suspected Head and Neck cancer pathway. Therole of the Cancer Pathway Navigator (Navigator) is to improve the efficiency and flow for patients on a suspected cancer pathway. The post holder will work closely with cancer services staff, clinicians (Consultants and Clinical Nurse Specialists), managers and booking staff in order to identify and resolve delays in the pathway. The Navigator will be involved from the point of referral for suspected cancer right through the patient's journey to the point when treatment starts. The post-holder will be responsible for co-ordinating patient appointments, ensuring the timeliness of investigation and monitor their progress along the patient journey. The Navigator will be pivotal in escalating operational issues and maximising available capacity by providing feedback to both clinical and non-clinical staff working towards achievement of the 28 day faster diagnosis and 62 day referral to treatment standards. The post-holder will also provide non-clinical support to the Clinical Nurse Specialists.,
- Navigate & chase diagnostic investigations and appointments for all patients on the cancer pathway within the appropriate timeframes
- To act as a first point of contact for patient queries in relation to their cancer pathway and next steps
- To proactively communicate with patients and families in relation to upcoming appointments and provide guidance around next steps and what to expect
- To be knowledgeable on all cancer waiting times standards and enable the implementation of timed pathways
- Work closely with the relevant MDT co-ordinator as required
- Work collaboratively with the relevant Clinical Nurse Specialists (CNS) to provide a seamless service, which is responsive to patients, carers and relative's needs
- Attend cancer PTL meetings
- Attend the weekly MDT meeting, taking actions where required
- Help to develop and maintain office and administrative systems and processes to support the work of the cancer team
- To be confident in suggesting improvements or additions to better serve our patient-focused service
- Regularly review and analyse waiting times across cancer pathways, escalating issues where necessary
- Work closely with the relevant MDT co-ordinator to act as a point of contact for clinical staff, managers and booking team for issues related to the relevant cancer pathway
