Full Job Description

An opportunity has arisen within the colorectal department for a motivated and dedicated Cancer Pathway Navigator. Reporting to the leadership and management team and working closely with the Clinical teams and MDT co-ordinators, the post holder will be responsible for ensuring that deadlines are met and that there is a pro- active co-ordination of patient pathways to ensure appropriate treatment is received in line with national and local targets., The post holder will be the key person identified for providing information and support to the MDT with key stages of the patient's pathway, to enable people to navigate the health and social care system. The aim of the role is to make sure that people living with cancer can access the right support, at the right time. Responsible for joining up services, coordination, providing information and improving communication between care givers, treatment providers and their families, to create a seamless patient experience.

The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.

The purpose of this role is to complement the existing multidisciplinary admin team and support patients through the diagnostic pathway and treatment pathways, if a cancer is confirmed and follow up.
