Full Job Description

You will be working as part of a small, friendly multidisciplinary team to deliver a universal prehabilitation telephone triage and sign posting service, and you will support in delivering group-based exercise and health and wellbeing education for patients who are living with and beyond cancer. You will be able to work flexibly with the ability to drive to venues within Swindon and the surrounding areas, as well as office and home working.
As part of this pioneering team, you will also be supporting health and wellbeing workshops both virtual and face to face and collaborating with the team to look at new proactive ways to improve wellbeing and access to rehabilitation to those affected by cancer.
Finally, you will be an integral part of this developmental team to showcase the benefits of health and wellbeing and rehabilitation as part of the Cancer Patient Pathway in improving quality of life, physical outcomes and empowering patients to self-manage and live active, healthy lives.
This service operates 8.30-4.30 Monday to Friday and does not require bank holiday working.

To deliver group based prehabilitation and rehabilitation for cancer patients as well as 1:1 clinics which may be face to face, virtual or telephone appointments.
2. To deliver health and wellbeing education via both virtual and face to face workshops on cancer related topics.
3. To collaborate with a wide range of the Cancer Services multi-disciplinary team as well as patients, family members and carers around the management of patients and development of the Personalised Care Team.
4. Develop skills and knowledge through participating in continuous professional development, in-service training and any other developmental activities.
5. To undertake basic physiotherapy and occupational therapy assessments that are appropriate to the patient as part of group based and 1:1 rehabilitation.
6. To carry out delegated tasks given by the physiotherapist, occupational therapist and nurses.
7. To help plan, set up and deliver health and wellbeing and rehabilitation workshops/services.
8. To deliver a telephone universal prehabilitatation triage service, to educate newly diagnosed patients on how to improve their overall fitness and health and wellbeing before, during and after starting cancer treatment.
9. Sign-posting patients to local and online services to promote self management either for prehabilitation or to manage post treatment side effects.

Candidates are responsible for covering the cost of their own DBS check if required: £18 for a Standard DBS check, recoverable from the first pay, or £38 for an Enhanced DBS check, divided and recoverable over the initial two pay periods. Payment for DBS checks for bank posts must be settled prior to commencement. Apprentices are exempt from DBS payment.

Fixed term for 12 months, full time 37.5 Monday - Friday
We have an exciting opportunity for a one-year fixed term contract or secondment for a band 4 to join an innovative, Macmillan Therapy Team at Great Western Hospital Trust. You should have relevant experience working as an Occupational Therapy or Physiotherapy assistant, support worker or equivalent in a healthcare setting.
This new Band 4 Rehabilitation Assistant role is a full-time post and will be supported by a Band 7 Occupational Therapist and Physiotherapist through a supervision and appraisal process.
We will offer training and support to develop your existing experience to deliver both group-based rehabilitation, prehabilitation and 1:1 clinics, as well as supporting health and wellbeing events for cancer patients.
There are also opportunities for paid study leave and/or course funding both internally/externally.
