Full Job Description
Cancer Support Workers (CSW) operate as part of the cancer care team to improve care for people who have a suspected or confirmed cancer diagnosis.
The CSW will work under the direct supervision of the Dermatology nursing team at MCHFT., The Post holder will be responsible for the check of Rapid Access referrals ensuring that all relevant information is present and ensuring the patient is on the most appropriate pathway. You will liaise with the GP practices and any other services to ensure that all relevant information is collated.
You will be required to work across a range of IT systems to coordinate the booking of patient appointments.
You will work both directly and indirectly with patients and their carers to assist in coordinating the care of patients through their hospital care journey, providing the patient with the relevant information and coordinate any test preparations that need to be sent or collected by the patient.
The post will require the coordination of appropriate services to maximise the delivery of streamlined diagnostic pathways, thereby improving quality of care and experience for patients and their carers, and diagnosing cancer earlier.
Mid Cheshire Hospitals NHS Foundation Trust provides a full range of hospital and community services for people across East Cheshire (population 399K) and West Cheshire and Chester (population 357K).
