Full Job Description

The Patient Directed Open Access (PDOA) Service provide on-going support and surveillance to patients who have completed their primary cancer treatment & deemed eligible to move onto a self-management pathway.
Remote Health Cancer Support Workers play a vital role in the PDOA Service providing essential assistance, surveillance and emotional support to individuals and families affected by cancer.
We are looking for a compassionate, enthusiastic & organised Remote Health Cancer Support Worker to join our team who will be required to act as the first point of contact for patients and work closely with clinical teams to provide personalised support to patients, monitor on-going protocolised surveillance & support patients living with and beyond cancer.
The ideal candidate will have experience in oncology, strong communication skills and a good knowledge of administrative systems., + To act as the main point of contact for stratified patients
+ To provide complex, but non-clinical information to patients and carers
+ Facilitate and lead regular workshops to educate patients in navigating Personalised Stratified Follow-up (PSFU) self-management pathways
+ Use initiate to sign-post patients to relevant services and support
+ Fostering links between patients and their key workers and specialist nurses
