Full Job Description

Within a multi-disciplinary environment, deliver a comprehensive rehabilitation programme of secondary prevention of cardiovascular diseases and management of Long Term Conditions. The programme will be delivered within the community setting as part of the Wirral Community Cardiology Service. The post holder will also support the development and supervision of junior staff and students from a range of backgrounds and disciplines. This role will be pivotal in ensuring that the service is able to deliver against all of its quality and activity key performance indicators.

Main duties of the job
The post will require the individual to work as a part of the cardiac rehabilitation team to facilitate the current cardiovascular and heart failure rehabilitation programmes, including the facilitation of exercise and education sessions both at the St Catherine's Health Centre, Birkenhead and Victoria Central Health Centre, Wallasey sites. They will cover Wirral Intermediate Cardiac Clinic as required. The post holder will also manage a complex clinical caseload of patients with cardiovascular disease, heart failure and cardiac arrhythmias. Be competent at assessing the patient at entry to the programme and co-ordinating their care through to discharge using a patient led goal setting model. The post holder will have extensive knowledge of cardiovascular disease and heart failure, plus knowledge of treatment pathways and cardiac pharmacology.
Excellent communication skills and an ability to work in a team and to direction is essential. Team work is a major feature of this role and the ability to work and communicate with a diverse and multidisciplinary team and as a senior member of the team take on a leadership role, supporting less experienced team member and supporting the management team in developing the services we provide. The post will involve covering colleagues work during periods of absence and will include some evening work.

Provide high quality evidenced based community cardiology services, responding to appropriate policy direction, particularly supporting delivery of:
NICE guidance relating to the management and prevention of Cardiovascular Disease
1. NICE Guidelines on Heart Failure Management
2. NICE Guidelines on Atrial Fibrillation
3. NICE Guidelines on Hypertension
4. Local Delivery plan
5. NHS Long Term Plan
Ensuring public and patient participation within service planning context, plus provide a well-trained and professional workforce., Within a multi-disciplinary team assist in the co-ordination and delivery and development of a comprehensive cardiovascular rehabilitation, to include:
+ Workload prioritisation planning and coordination of patient care on a day by day basis, setting and maintaining highest clinical standards.
+ Network with other multi-disciplinary, statutory, private and voluntary organisations, users, carers and assisting in managing the interfaces between them.
+ Contributing to the training needs analysis plan.
+ Record compliments and complaints of service.
+ Deputise for Lead Cardiac Rehab Nurse as required.
+ To ensure seamless continuity and transfer (if appropriate) of care for patients between other relevant agencies/professionals and other care settings.
+ To act as a role model for other clinical team members, establishing positive and credible professional leadership and be involved in mentoring of staff and clinical supervision as appropriate.
+ If appropriate assist the Cardiac Rehab Leads in the recruitment and selection process.
+ Provide mentorship for nursing students as per NMC guidelines.
+ In conjunction with other members of the multi-disciplinary team, review existing protocols and policies and develop new ones where appropriate.
+ Within the framework of clinical governance, work continuously to improve quality and safeguard standards of care, maintaining a patient focused service, promoting and implementing evidenced based practice with measurable standards of care and outcomes.
· To be responsible for the training, supervision and mentoring of all junior Cardiovascular team members and students within the department.
+ Attend and represent team on regular relevant meetings, as agreed by the team (including Trust, local, network) and provide effective channels of communication within the team.
+ Report risks and incidences for the service on Datix.
+ Maintain a flexible approach and develop the role according to need.

Clinical
· Managing a highly complex patient caseload, including the preparation and completion of patient assessments to establish physical, psychological and educational needs and goal setting plans including families and carers and being responsive to any communication barriers.
· Manage highly complex patients who may have significant physical and/or psychological needs and managing the imparting of unwelcome news to patients in relation to their health and/or diagnostic results within scope of practice
· Facilitating the deployment and organisation of core cardiovascular rehabilitation to include educational talks, exercise and relaxation classes and training of newer exercise physiologists and cardiovascular rehabilitation nurses in the appropriate elements.
· Ensuring adequate levels of supervision of all cardiovascular rehabilitation exercise sessions in line with service policies and procedures and according to the patient's care plan.
· Ensuring that all patients progress through the relevant stages of the cardiovascular rehabilitation programme at the correct time and that appropriate information and advice is provided at each stage
· Assuming responsibility for monitoring the progress of all patients undergoing cardiovascular rehabilitation, evaluating their condition and adapting their programme as necessary within department standards and protocols, seeking assistance as required.
· Preparing the exercise areas and maintaining clinical equipment, acting on any faults immediately and liaising with service contractors and/or Heart Support Manager.
· Ensuring the daily cleaning and regular deep cleaning of all exercise equipment as per department standards.
· If appropriate assist in the running of prescriptive and diagnostic Exercise Tolerance Tests as per department policy and procedures.
· Continually assess patients' suitability for exercise-based cardiovascular rehabilitation and liaise with relevant health professionals for further advice as required.
· To maintain and provide relevant statistics about activities as required by Trust, District, Network and National Organisations.
· Work within the Wirral Community NHS Trust Policies and Procedures.
Professional Development and Education
+ To be responsible for the continuing update of own professional knowledge and maintain individual professional development in line with PREP and NMC requirements, undergoing regular Individual Performance Reviews.
+ Work closely with all members of the multi-disciplinary team to identify training needs for all staff and participate in the development, delivery and evaluation of appropriate multi-disciplinary teaching programmes in conjunction with other relevant professionals, departments and local University school of Health Studies.
+ To maintain close links with the Trust Professional Development Department, + Participate in or undertake research projects, as identified and disseminate research findings appropriately, maximising opportunities to promote research in practice.
+ Conduct and participate in audit to evaluate identified aspects of the Cardiovascular Rehabilitation Service, using techniques that monitor and improve existing practice.
+ To maintain close links with the Trust Quality and Governance Department.
