Full Job Description
31.5 hours over 4 days between the hours of Monday to Friday 8.30am to 5pm.
Base: Between Clwydian House, Wrexham Maelor and Connah's Quay Health Centre, Flintshire.
An opportunity has arisen for suitably trained, enthusiastic & motivated individual to join the BCU East Cardiac Rehabilitation Team on a permanent basis.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
The support worker will work as part of the team alongside the nurse, occupational therapist, exercise physiologist and administrator in order for the patient and family to receive appropriate Cardiac Rehabilitation. The successful candidate will join the team in delivering care in various community settings - leisure centres, patients' homes, community hospitals and surgeries.
The individual will use a number of skills to suit the variety of the job that will involve direct patient care, office work, telephone contacts and following protocols. Travel will be involved within Wrexham and Flintshire areas.
The team can be very busy and the individual must be able to use their own initiative and be flexible to aid with the smooth running of this rewarding service. The individual should be qualified to QCF level 3 eg NVQ or ONC related to health & social care or exercise. Further support & training will be given to meet the needs of this role.
