Full Job Description
An exciting opportunity has arisen for a secondment as Cardiac Triage Nurse CNS.
The post holder will be a clinically based competent practitioner who will take a supportive role in strengthening the nursing contribution to patients and service users within the specialty across the organisation.
In partnership with multidisciplinary teams (MDT) across the Trust the post holder will assist in developing, implementing and evaluating a seamless service for cardiology patients across the Trust, and in particular the Emergency Division.
Developing enhanced nursing skills; you will contribute to the management of patients which will include assessing patients, ordering investigations, reviewing results, undertaking clinical procedures, identifying patients and ensuring appropriate pathways are instigated and relevant review/transfer to and from the specialty.
The post holder will contribute to the on-going development of innovative practice, standards of care and strategies to ensure practice development and education for other professionals caring for patient within the specialty., Contribute to the delivery of evidence-based personalised and compassionate care which places quality at the heart of practice. Care will be responsive to individual needs and reflect current guidelines.
Be an accessible professional for the multi-disciplinary team (MDT). This will necessitate a degree of autonomy and advocacy to ensure the delivery of appropriate care.
Use knowledge of the specialty and treatment to support the development of personalised care pathways for individuals and assist with holistic needs assessment, planning and evaluation of care and all care interventions.
To utilise advanced cardiac nursing skills to conduct in depth patient assessment and history-taking in order to plan & implement cardiac care for patients reviewed along the care pathway.
Act as a patient advocate to facilitate the process of shared decision making in respect to health.
Provide specialist advice regarding cardiac disease and symptom management for patients within the specialty in both inpatient and ambulatory settings. Evaluate response to interventions.
Support patients to self-manage their condition where deemed appropriate. Provide education for those who will need guided support to self-manage and those who will need on-going face to face support.
Act as a resource providing specialist advice and support across service boundaries.
Undertake clinical competencies relevant to this service to demonstrate expertise in extended nursing roles., 1. Act as a role model demonstrating high standards of holistic care and provide clinical leadership to others across the organisation.
2. Participate in the recruitment and selection of staff where required.
3. Provide support to ensure public and patient involvement is embedded within the sphere of practice.
4. Assist in setting measurable outcomes for the service and continuous evaluation of practice supporting changes where necessary.
5. Participate in operational and strategic planning for the development and delivery of the service. Support service improvement activities.
6. Attend Trust senior nurse meetings and contribute to the agenda and strategic nursing vision.
7. Represent the Trust by contributing to and participating in meetings locally, regional and nationally relating to the specialty.
8. Contribute to an annual service review, gathering information to support the annual report.
