Care Co-ordination Team Leader

You will oversee the integration of services by working collaboratively with GPs, District Nurses, Social Workers, and other professionals to create seamless care plans that reduce hospital admissions and promote patient independence.

Key responsibilities include managing your team's performance, ensuring compliance with governance standards, and fostering a culture of continuous improvement. You will use your leadership skills to inspire and motivate, ensuring timely, effective communication between all stakeholders.
