Full Job Description

The Right by You service has been designed and shaped by people with lived experience of cancer. Our aim is that the service should work for anyone who needs it. Over the past couple of years the project has been working with people who are experiencing homelessness (or at risk of homelessness) who are diagnosed with cancer. To help us progress this work we are looking for a link-worker with skills and experience of working with people who are homeless to join our Right by You team in Southampton. The post is being hosted by the Homeless Healthcare team but will work closely as part of the Right by You project team. The individual will be supported to learn about cancer and cancer support and will be part of a growing team of dynamic practitioners delivering meaningful personalised care wherever it is needed

The Care Co-ordinator will be a key point of contact for the homeless population within Southampton who have cancer. To help meet their needs and provide co-ordination and support.

The post holder will work closely with the Right by Your Colleagues, Homeless Healthcare Team and Street Homeless Prevention Team to manage an identified caseload of patients to support them to achieve their goals, access treatment and support to improve lifestyle.

Main duties of the job

Assist patients to access self-management education courses, peer support or interventions that support them to take more control of their health and wellbeing in relation to the PCN Service Specifications.

Provide co-ordination and navigation for patients and their carers across health and care services providing intensive support via assertive outreach to people who are homeless or at risk of homelessness in the city maintaining a consistent approach.

Work with other health and social care providers to identify and manage a caseload of patients which could involve the exchange of complex information.

Accompany chaotic and complex clients to interviews and appointments and facilitate relationships and support via other members of the Right by You team and other cancer services.

Support patients to attend appointments through answering queries, arranging and managing appointments and ensuring that they have good quality written or verbal information.

Engaging and supporting rough sleepers on the street working in conjunction with colleagues from the Homeless Healthcare Team, Street Homeless Prevention Team and the Right by You Wessex team.

Manage a caseload of clients maintaining clear and accurate records of the support offered and outcomes achieved.

Complete an initial assessment to identify client needs and produce person centred outcome and holistic support plans.

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
