Care Co-ordinator

The Klinik triage team is the heart of The Adam Practice. The main aim is managing the day to day workload of patient appointment requests via reception teams and online consultations.

To be responsible for the day to day administration of Klinik request. Ensuring a timely and efficient turnaround of appointments.

To carry out all duties in a professional manner and adhere to all Adam Practice protocols, policies and guidelines at all times.

To have excellent communication skills, with the ability to communicate in writing and verbally to a wide range of providers and users of the services, including patients; carers; voluntary; social; primary and secondary care providers.

The Adam Practice is a partnership located across 5 surgeries in Poole, Dorset. This is an exciting opportunity to join our growing primary care team in a forward- thinking, innovative, general practice, which has a current CQC rating of Outstanding in Care.

Here are some of our many benefits we offer

Wednesday - 09:00 - 17:30 - 8 hours (not including a 30 minute unpaid break)

Thursday - 13:30 - 17:30 - 4 hours

Friday - 13:30 - 17:30 - 4 hours

Total - 16 hours, Access to NHS Pension Scheme and NHS discounts, 5 weeks annual leave (inclusive of bank/public holidays that fall in the holiday year). Entitlement is pro-rata for part-time employees.
