Full Job Description

The Perinatal Mental Health Team are excited to recruit a permanent, care coordinator (nurse/social worker/OT) to work full time as part of the multi-disciplinary clinical team. 30 Hours per week (0.8 WTE). The team is accredited by the Royal College of Psychiatrists. It is a busy, secondary care mental health service that offers a range of evidence based and NICE concordant mental health interventions to the women of Worcestershire during the perinatal period. The team is passionate, motivated and collaborative and operates on a Recovery based person-centred ethos. Professional development is encouraged; there are strong links with local and regional CPD and training networks, with regular trainees on specialist placements. This post offers a fantastic opportunity to gain experience working in a multi-disciplinary team of highly-skilled and knowledgeable clinicians. The successful candidate will ideally be able to demonstrate experience of working in a perinatal or mental health related field, or alternatively will be able to demonstrate in interview, a thorough understanding of the specific role a nurse/OT/social worker will fulfil in a perinatal service pathway. It is essential that applicants can meet the travel requirement for the post. The service operates from 9am-5pm, Monday to Friday., The post holder will work in support of the close MDT within a community mental health team. Supervision and teaching of junior staff will be part of the role. As a team we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice. The post holder will be responsible for completing assessments, supporting with groups and working alongside all members of the MDT as well as patients, their families, carers and infants., We want to ensure that patients needs are met and families are supported. We work with a number of partners, including GPs, Social Workers, Midwives and Health Visitors, to ensure this is the case. We offer home visits, clinic appointments, virtual appointments and a wide range of groups to our antenatal and postnatal mums and partners.

The Perinatal Mental Health Team is a specialist community-based service focused on helping women at risk of developing, or experiencing moderate to severe mental health difficulties, during pregnancy and during the 2 years following childbirth or require pre-conceptual counselling., At Herefordshire and Worcestershire Health and Care NHS Trust, we are working together to deliver outstanding care.
