Full Job Description

We seek a temporary Care Co-ordinator to cover maternity leave at our Primary Care Network.

As a Care Co-ordinator, you will use health data to identify and support patient groups, delivering personalised care plans. You will assist patients in preparing for shared decision-making and ensure they have the information needed to manage their health.

Your role includes coordinating care needs into a single plan, helping patients with appointments, and providing access to training, employment, and benefits. You will guide patients to self-management resources and personal health budgets where suitable.

You will work closely with health professionals to navigate and coordinate care services, supporting multi-disciplinary teams. Additionally, you will promote shared decision-making within the network, helping staff and patients prepare for these important conversations.

You will work within Tower Network, the largest Primary Care Network within Tower Hamlets, covering the entire South East of the borough, covering over 80,000 residents.

This is a fixed term role to cover maternity leave, for three days (22.5 hours) per week.,

  • Utilise population health data to identify and engage with specific patient cohorts.

  • Develop and maintain relationships with patients to understand and address their unique health needs.

  • Create and manage personalised care and support plans (PCSPs) that reflect patients' preferences and best practices.

  • Ensure holistic integration of all identified care and support needs within the PCSP.


  • Support patients in using decision aids and preparing for shared decision-making conversations.

  • Promote and facilitate shared decision-making within the Primary Care Network (PCN).

    Tower Hamlets GP Care Group (The Care Group) was incorporated as a community interest company limited by shares in September 2014. Every general practice in Tower Hamlets owns one share of £1. The Care Group was formed to manage borough level contracts on behalf of the practices, deliver primary care led services and support general practices, and to be the provider voice of primary care in the wider health and social care system.
