Full Job Description

Whilst this role is based at Orchard, the successful candidates will work closely with other PCN staff, including the CCO at Park Surgery. We have a higher than average population of elderly patients, many of whom live in their own homes and need a lot of social and clinical support. As a PCN and practice, we will be focusing on developing a Frailty programme for these patients, to take a more proactive preventative approach to caring for them. We also have a focus on Womens health and are working in our local community to improve access for women through Information Sessions. We have a strong focus on dementia, young peoples mental health and are keen to develop these further. The PCN personalised care team will have the full support of a clinician experienced in supporting the personalised care roles and will receive monthly individual clinical supervision as well as group learning. There will be opportunities to undertake the 2 day Personalised Care Institute accredited care coordinator training and to achieve the competencies set out in the NHSE Care Coordinator Development Framework.,

  • The Care Coordinator is central to the multidisciplinary team within the practice and our Primary Care Network. They will be the first point of contact for patients and service users with complex needs, long term conditions or frailty, providing an in depth knowledge of practice and local community services.The Care Coordinator will liaise closely with their clinical team, the personalised care team and management teams to ensure patients receive timely and appropriate direction or appointments according to their healthcare need. Responsible for ensuring cohorts of our population receive appointments in practice or at home for monitoring of long term health conditions. This aspect of the role may include other administrative duties as requested by the management team or partners.The care coordinator will be central to the Frailty service offered by our PCN working closely with Park Surgery Care Coordinator, our PCN management team, the Clinical Directors and clinicians.   • PCN, service development support will form a key part of the role.

    Orchard Surgery is a long established small friendly practice in the centre of Horsham. We have a population of around 9000 patients. We have a strong team of GPs, paramedics, clinical pharmacists, social prescribers, health and wellbeing coaches along with supportive managers and administrative staff. We value our personalised care roles. We form part of Park and Orchard PCN and together we have a population of 30,000 patients.
