Full Job Description
Our office was established in Dorchester with a mission is to brighten the lives of seniors giving them a sense of purpose, wellbeing & worth. This role will play a vital part ensuring that our clients receive the best in class quality care and support the growth of our business., The main purpose of the Care Coordinator is to be responsible for all tasks related to arranging client scheduling. This includes matching and scheduling care professionals to clients. This role would suit an individual who enjoys solving puzzles, arranging logistical solutions and dealing with problem solving. As the Care Co-Ordinator, you will;
- Assist with the onboarding of new joiners
- Coordinate the forward planning of care schedules and ensure it is consistently achieved in order to provide continuity of care to our clients
- To support the Registered Care Manager and the Care Supervisors to ensure that any communication with external sources such as next of kin, GP's, nurses, physiotherapists and any other professionals involved is prioritised if they impact on the care and wellbeing of the Client
- Ensure that all Care Professional holiday, sickness, unplanned absences and all emergency calls are documented electronically;
- Covering care calls and gaps in the care schedules where they arise and cannot be covered
- Be part of the care team on call rota system during evenings and weekends as and when required
- Understand the arrangements for ensuring all clients are safeguarded against the risk of abuse and respond appropriately to any allegations of abuse, or complaints regarding service delivery whilst complying with Home Instead policies and procedures.
Need to have a high level of attention to detail gained in a previous customer service orientated role; - Require a strong knowledge of IT systems
- Be self-motivated and demonstrate integrity, trustworthiness, reliability;
- Ability to build and maintain strong working relations with clients, care professionals and colleagues alike;
- Previous experience working within care is preferable, but not essential;
- Experience of co-ordinating rotas or schedules is also desirable, but not essential.
- Hold a valid driving licence and have use of your own vehicle.
