Care Coordinator

We are seeking a dedicated and experienced Care Coordinator to join our growing team. This role is essential to ensuring that our clients receive the highest quality of care and that our support workers are well-supported and motivated., As the Care Coordinator, you will act as the primary point of contact for clients and support workers, providing critical operational support and overseeing daily care logistics. Key responsibilities include developing and managing care schedules, monitoring and addressing cancelled care hours, overseeing high-quality, person-centred care, hosting team meetings, recruiting and matching support workers, collaborating with internal and external stakeholders, supervising care and medication standards, and planning and deploying resources efficiently across care packages.
