Full Job Description

An exciting opportunity has arisen for a motivated and skilled Mental Health Nurse, Occupational Therapist or Social Worker to join our Early Intervention in Psychosis Team in Wigan as a Care Coordinator. The main aim of the Early Intervention Team is to offer treatment for individuals, aged from 14 - 65 years, who are experiencing a First Episode of Psychosis in line with NICE guidelines. We achieve this through offering: - Care Coordination. - Evidence-based group and individual psychological interventions, including Cognitive Behavioural Therapy for psychosis and Behavioural Family Therapy. - Treatment, support and education regarding medications. - Regular physical health monitoring. - Support with accessing employment, education and vocational activities. - Opportunities to engage in social activity and physical health promotion. - Support for families and carers. With line management and clinical supervision from within the Team, the post holder will work as part of the integrated MDT, providing clinical interventions. These will focus on relapse prevention, improving quality of life and collaboratively working towards recovery. The post holder would be expected to maintain and co-ordinate timely and appropriate interventions through assessment for a caseload of service users. This includes respecting individuality, values, culture and religious diversity of service users and colleagues.

  • To hold and manage a caseload of service users who have
  • been identified as experiencing their first episode of psychosis, including needs assessments, care planning and delivering psychosocial interventions.
  • To complete specialist assessments for referred service
  • users accepted into the service when required.
  • To undertake risk assessment and risk management for
  • individual service users and to provide advice to other professionals on acute and chronic aspects of risk assessment and risk management.
  • To work as part of our integrated MDT, and to participate in
  • a clinical decision making process to determine the most appropriate management plans for all service users who are under the care of the team.
  • To participate in the duty rota operated by the Early
  • Intervention Team.
