Full Job Description
The Lambeth Early Onset Team (LEO CMHT) are looking for a highly enthusiastic Care Coordinator to work within Care Coordinator role. We welcome applications from CPN's, OT's and Social Workers. We are looking for someone who would like to develop their knowledge and experience within the area of First Episode Psychosis and work within a large and busy multi professional team. The main role of the Care Co-ordinator will be to develop a range of interventions to aid the patient in understanding their psychotic experience, and developing skills and knowledge to reduce the risk of relapse, but also to identify and use early warning signs, to enable early detection and intervention. The Care Co-ordinator is also responsible for working with the patient to develop a package of care that encompasses all their needs, and to aid in developing opportunities and accessing services as appropriate for the patient. This may include housing, benefits, employment, Citizens Advice Bureau, refugee organisations etc. The Care Co-ordinator will also provide regular feedback to the Multidisciplinary Team and the patient's GP regarding the patient's progress. Qualified RMN, OT or Social worker with current aligned professional registration (i.e. NMC, HCPC) Evidence of continuing professional development e.g. relevant training Significant post registration experience in a mental health setting Experience of working in a community setting Experience of working with service users with psychosis Experience of the use of evidence based interventions relating to the care of service users with psychosis Knowledge of current Government Policy and Guidelines concerning the provision of community mental health care in general and of Early intervention in Psychosis in particular Excellent verbal and written communication skills Ability to provide care that is sensitive to the needs of minority groups Understanding of the supervision process
