Full Job Description

You will provide an efficient, confidential administrative service to the Urgent Community Response Team, you will be based at Wymondham Health Centre.,

  • Care Coordinator for the Urgent Community Response Team in the South Locality assisting with all aspects of general administration tasks, to support an admission avoidance team. Helping to manage referrals and allocation in a very busy office, with a caseload of patients constantly changing. This Service provides rapid assessment and short term care for people who would otherwise be admitted to hospital and also facilitates early discharge from hospital.
  • The team has registered therapists and nurses who work alongside assistant practitioners, health care assistants and support workers to enable people in our care to live as independently as possible. We work very closely with GPs, Social Services and other agencies.

    You will need to have previous administration and secretarial experience, as part of a team and within a busy office environment.
  • Applicants must have excellent organisation, communication and customer care skills and be able to work proactively using own initiative, demonstrating self-management and prioritisation of their workload.
