Care Facilitator

The purpose of this role is to provide high quality holistic support to older people to allow them to remain living independently, safely and well at home. Working closely with GP surgeries, Multidisciplinary Teams and Frailty Teams across Newport, you will take a holistic approach to support.

Support will be person centred and will be based on a 'Stay Well Plan' which focuses on all aspects of their daily life. The plan will help identify areas where support may be required, and will allow you to actively promote a client's wellbeing as well as signpost to any internal and external relevant services to create resilience and to support them to remain living independently in their own home.
