Full Job Description

As a Care Manager in our public-facing team Information and Advice Team, you can make your own powerful contribution to adults with complex needs who require support from our outstanding Social Care team.

As the first point of contact for those who have health and social care needs (plus those who are being discharged from hospital), you'll act as the gatekeeper for our services, completing comprehensive screening assessments so that you can determine what our next steps should be. Providing a compassionate and empathetic level of support to every adult that you are working with, you will provide them with advice and information, and signpost them to third sector organisations and partner agencies. Seeing to it that the relevant legislation is being considered at every turn, you will have the chance to implement creative interventions, so you will be encouraged to consider options such as assistive technology. If an adult does not meet our eligibility criteria, you will explain why to their referrer, and make suggestions about other more suitable pathways.

The wellbeing of our local community underpins everything that we do. Where you identify risks, you will put the relevant safeguarding measures in place (including making onward referrals and determining whether a full safeguarding enquiry is required) and in doing so, you will see to it that our customers are able to live fulfilling, safe and independent lives wherever possible.

As a qualified Social Worker, Occupational Therapist, Nurse or Care Manager with experience of conducting assessments of physically disabled or older people, you will ideally have knowledge of working within a Social Services Duty System - so you will also have an in-depth knowledge of the relevant legislation. Crucially, you will bring an enthusiastic approach to every day, and you will enjoy coming up with innovative solutions to complex problems. From a practical point of view, you will also have the capacity to use our computer data base (Mosaic).

As Safeguarding Adults Manager, Clinton knows how important it is to remain objective and look at all the evidence. But it was his love of TV detective programmes that made him think again in the case of B, an adult with learning disabilities. For years, everyone had believed B was living with a relative. But his so-called carer turned out to be a 'cuckoo' who had taken over his home. B had finally found the courage to speak out about the psychological and financial abuse he'd endured.

Through his investigations Clinton deduced they were dealing with not one but two vulnerable adults. It was no good simply removing the alleged abuser from B's life. Unusually, he too needed support and advice. This was a complex case. It meant working with multiple services, from the local police to healthcare to housing, but the wellbeing of B and his unwanted house-mate were at the centre of every decision., Westminster is an amazing place.
