Full Job Description

We are looking for a dynamic and tenacious person to join our team. You may have experience of working in mental health but also possess robust admin skills or you may be an experienced administrator looking for a more clinical facing role? You will be flexible, adaptable and able to use your own initiative to benefit the discharge pathway of our patients. Tasks may include liaising with clinical teams, directly supporting patients to identify and address barriers to discharge and supporting the flow lead with practical tasks such as booking transport, making referrals and maintaining accurate records.

The aim of the Care Navigator role is to deliver high-standards of care and pathway management to reduce delayed transfers of care across the trust. You must have the ability to communicate effectively with the multidisciplinary team whilst having the skill of providing reassurance for both patient and their families during what can be a worrying and anxious time when discharge planning.

This post requires you to be working on site at Antelope House. There may be opportunity for home working but with the expectation that you would be on site for at least 70% of your working week. The service works across 7 days per week so there would be some weekend working on a rota basis, this will offer enhanced pay. Antelope House is a 54 bedded mental health hospital in the centre of Southampton. We have free onsite parking and you would be offered the relevant training and support to undertake this role., Ability to work as part of a team liaising with the MDT regarding discharge planning for patients and the timely early identification of potential discharge delays. To assist in communicating the discharge process to the patient but not considered responsible for making the recommendation request.

Ability to work independently to facilitate discharges in a timely fashion and to highlight any concerns that may prove to be a barrier to discharge.

Ability to liaise sensitively with patients, family and/or carers concerning issues related to discharge planning that are appropriate to the grade.

Liaise with other agencies, patients, family and/or carers in order to provide effective communications to assist with timely discharge of patients.

To have an awareness of patient flow throughout the hospital and impact of discharge within this.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
