Full Job Description

Within this role you will be responsible for arranging packages of care for Adult Social Care clients with care providers when directed. You will be arranging clients care to be provided by a bridging provider if sufficient hours available to facilitate hospital discharge and monitor bridging on a daily basis. Providing a high level of customer service, you will create and send to providers the contract order form and be responsible for the timely and accurate recording of all orders for all care packages commissioned. In addition, you will liaise with Adult Social Care and providers to resolve queries and make amendments to care packages.

You will have good interpersonal skills and confidence in dealing with and liaising with practitioners and external care providers. With a strong attention to detail and the ability to plan and prioritise your workload, you will be able to work autonomously whilst under pressure and to deadlines. A team player you will have knowledge of the process of raising purchasing orders and supplier invoicing and be competent in the use of Microsoft Office applications.
