Full Job Description

As a Field Care Supervisor, you will play a crucial role in ensuring we deliver the highest quality care support services. Your responsibilities will include:

  • Supporting Care Workers: Provide practical supervision, instruction, and support to our dedicated care workers to ensure they meet the highest standards of care.
  • Assessment and Review: Conduct assessments and reviews of individual services, including regular needs and risk assessments, to ensure our service users receive the best possible care.
  • Care Planning: Collaborate with Care Coordinators to plan effective and realistic care worker rotas that align with the specific needs of our service users and the skills of our care workers.
  • Performance Monitoring: Perform on-site observations and evaluations of care worker practices as part of routine supervision or in response to performance concerns.
  • Multi-Disciplinary Collaboration: Participate in multi-disciplinary reviews of service users' needs to provide comprehensive and coordinated care.
  • Training and Mentorship: Shadow new care workers in the field to ensure their competence and confidence in delivering exceptional care.
  • While the majority of your time will be spent in the community, you'll also handle some office-based administrative tasks to support our care operations.,

    Assessment Experience: Proven experience in conducting initial risk and needs assessments for individual service users, whether in their homes or in other settings such as hospitals, residential, or nursing homes.
  • Collaborative Approach: Ability to work in partnership with service users, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive, holistic care packages.
